　　Good afternoon, sir. The Doctor is here today. Do you have an appointment?
　　下午好，先生。医生今天不上班。你预约了没有？
　　No, I don't. A friend of mine, Mr. Henderson, recommended Doctor Smith.
　　没有。 我的朋友亨德森先生介绍我来看史密斯医生的。
　　I want to make an appointment. My name is Mr. Turner.
　　我想要预约一下。我的名字叫特纳。
　　I see. Wait just a minute. Doctor Smith, there's a patient here. He's a friend of Mr. Henderson. He wants to make an appointment.

　　I'm not busy now. Ask him to come in. Hello, my name is Doctor Smith. What can I do for you?

　　I have a tooth that's giving me trouble. It's this one, the upper front tooth.
　　我有一颗牙有毛病。 就是这颗，上排的前齿。
　　When I close my teeth together it's rather painful. Pressure on it is very uncomfortable.
　　我把牙齿咬合时就感到很痛。把它压着可十分难受。
　　I can see that the gum is red and inflamed 1. There's also some swelling 2. When did you begin to have pain?
　　我看到牙龈已经发红，发炎了。什么时候开始感到痛的？
　　It began with a very slight, dull ache about a week ago.
　　大概一星期以前开始有钝痛的感觉。
　　The X-ray shows that the area around the end of the root has become infected.

　　This tooth has had a root canal filling. Do you remember when it was done X?

　　It was about three years ago.

　　Well, sometimes a tooth that has been treated endodontically will become infected or develop a cyst.

　　It doesn't always mean that the tooth will have to be extracted. It can usually be treated successfully by performing an apicoectomy.
　　并不是再这种情况下就一定要把牙拔掉。通常进行齿根尖切除就可以治好。
　　What's that, Doctor?

　　Apicoectomy is the removal of a very small Part of the infected root tip.
　　齿根尖切除就是要将受到感染的根梢的一个非常小的部分切除掉。
　　Another Part of the treatment is to remove the infected (or cystic) tissue from the jaw-bone surrounding the root.
　　另外，还要将牙根周围颌骨受感染的组织切除掉。
　　And what is needed for this?
　　这样做需要什么呢？
　　Sometimes a retrograde silver amalgam 3 filling is needed to seal the small opening in the root tip to be sure that the tooth won't become infected again.
　　有时需要用逆方向的方法填补上，银汞封蛀牙根根梢的小洞口，以便使牙齿不再受到感染。
　　Can you please explain how this is done?

　　An incision 4 is made in the gum just above the tooth at the level of the root tip.
　　就在牙齿上边，牙根梢那么点高的地方的牙龈切。
　　After the infected root tip and infected tissue have been removed,
　　再把受感染的牙根梢和受感染的组织切除之后，
　　a small undercut preparation is made in the root for a silver amalgam filling.

　　It's done in the same way that a small cavity is treated on the crown of a tooth.
